Sleep experts explain: Why many snorers continue to suffer despite aids

The possible consequences of sleep apnea     

Sleep apnea can impair concentration and mood .     

Every night, when breathing stops, the brain receives less oxygen for 10, 20, or sometimes 30 seconds.     

This is called hypoxia .  
And that can affect brain cells.

People with sleep apnea may experience changes in the areas of the brain responsible for memory, decision-making, and emotional regulation.

That's exactly what my patient Stefan felt for two years – he felt like he was losing his mind.     

He went into a room and couldn't remember why. He snapped at the children over trivial things. He couldn't concentrate in important meetings and almost lost three major clients. His wife, Julia, sometimes looked at him as if he were a stranger in his own home.     

But the effects on the brain were just the beginning.

Michael Berger experiences a turning point after 22 years.

Michael Berger has been a physiotherapy and sleep expert for 22 years.     

He has worked in several companies in Bavaria , cared for many patients and constantly furthered his education – with orthopedists and in courses on sleeping posture and breathing.     

He thought he had seen it all, until Stefan came to his factory one Tuesday morning.  Stefan looked like he'd had a rough night.     

Pale, tired eyes, shoulders hunched.     

"I dozed off at the wheel again," Stefan said quietly.     

"This time I hit my head on the steering wheel. 
Luckily it was just a minor scare and a laceration, but there was blood on the steering wheel."

The question that changed everything

Mr. Berger recommended a common aid to Stefan at the time – with a clear conscience.  Six months later, Stefan returned.     

Visibly dejected.     

"I tried it for six months with the device ," said Stefan

At that moment, Mr. Berger realized that there might be other approaches .     

Despite his experience , he had followed the usual guidelines and considered sleeping posture as a factor.     

He was familiar with the research on sleep posture, and like many colleagues, he had considered it – alongside the established CPAP therapy. But he had only considered it.     

“Stefan wasn’t just my patient. He shook me awake ,” Mr. Berger later admitted.     

"For years I had recommended tried and tested methods , and now I was looking for possible alternatives."     

He made a decision that would change things for both of them:     

"There could be another way."

The research that changed my view on sleep apnea

Stefan's case kept Mr. Berger busy for months.     

He finally decided to take a closer look at the studies on sleeping posture – something he was familiar with before, but now explored in greater depth.     

What he found in the data surprised him:     

Studies suggest that posture during sleep may play a role in many cases of sleep apnea.     

The respiratory tract can be affected by a specific position of the spine.     

A study by Stanford University investigated this:     

When patients with sleep apnea received adjusted cervical spine alignment , they showed a tendency toward improved respiratory indices —completely without devices. Here is the study.     

But that's exactly what got Mr. Berger thinking.

The simple reason that explains everything

Your breathing can be affected if your posture is not optimal .     
It could become narrower due to an unnatural position of the cervical spine .     

Imagine your airway like a garden hose .     

If you lie on a conventional pillow , your head could tilt forward or to the side.     

This could cause your neck to bend at an angle – potentially impairing your airway.     

CPAP machines can help force air through the airway. But they don't always do that.     

“We have tackled the problem for years using proven methods,” explained Mr. Berger. “Instead of just increasing the pressure, one could also consider the attitude.”     

This explains why some people are still exhausted despite good CPAP use .     

The airway may remain restricted.     

Your body senses that something is not optimal.     

That's why some men unconsciously remove their masks at night.     

The nervous system instinctively searches for a position that could support the airway.     

“Men who have difficulty with equipment are not undisciplined ,” Mr. Berger observed.     

"They are responding to a basic instinct."

Why conventional approaches may not address the root cause

Mr. Berger has tested common methods against biomechanical reality:     

CPAP devices? They can force air through the airway. They don't directly address posture. Disruption could remain.     

Bite splints? They can position the lower jaw, but they don't align the cervical spine. The neck could remain affected.     

Surgery? It can reshape tissue – but it doesn't change sleeping posture. A possible impairment due to the position could still occur.     

Studies suggest that in some patients who undergo surgery, sleep apnea may not completely improve.
 Here is one study.     

Positioning therapy (side sleeper training)? It can keep you on your side – but with a normal pillow, your head could tilt. This could result in continued discomfort.     

Losing weight? It can reduce tissue and helps in many cases, but it doesn't necessarily correct sleep-related misalignment. The mechanical element could remain.     

“Many recommended treatments do not always primarily consider the posture of the spine at night,” admitted Mr. Berger.     

“We look at symptoms, and it could be helpful to consider posture as a complementary factor – for better sleep.”

The expertise that is finally coming to light

What made Mr. Berger think :     

The role of posture during sleep is well-known in research.  

“In research, we have known for years how important the correct posture during sleep can be for the respiratory system,” admitted Mr. Berger.     

"But many manufacturers have not always implemented this so far."  
That changed when Mr. Berger heard about a company that was pursuing a different approach.
